Description: This Project includes: PROFESSIONAL SELECTIONS.  PROJECT TITLE:  SCI Smithfield – Replace Air Handlers BRIEF DESCRIPTION: The scope of work includes, but is not limited to replacing the air-handlers & radiators in the housing units, clean or replace existing ductwork, analyze duct size & as-built airflow/ventilation rates to demonstrate compliance with applicable codes & standards, upgrade the hydronic controls & provide new BAS. The primary focus of the project is the housing units, however, as funding allows, consideration shall be given to include the Medical Complex, Program Services building & Central Services buildings.

ADDITIONAL REQUIREMENTS OF SHORT-LISTED FIRMS:  Provide resumes for key design & management personnel; firm’s approach to project, incl. descriptions of tasks & services, & explanation of how approach provides value; a  description from firm’s portfolio of a project that best illustrates the firm’s ability to translate intentions into solutions that meet client needs & explaining how the final design translated into a meaningful, synthesized solution; an organizational chart & detailed work plan, identifying key personnel roles of the firm & consultants,  including a work plan identifying roles, disciplines & responsibilities, including percent of time assigned to project during design & construction & on/off-site participation of individuals; provide a description of your plan to manage quality for design phase, including a description of your quality management organization, activities to control quality for each deliverable, ensure interdisciplinary coordination & constructability throughout design phase; description of relevant projects completed by lead design professional, listing key personnel for each example; an assessment of key project issues & risks, including an assessment of schedule, budget, municipal approvals,  & other major issues that could be encountered & how they would be handled; identification of special investigations, studies or testing that might be anticipated; a description of how diversity will be accomplished within the team & confirmation of diverse & small business percentage commitments; evidence the firm is fiscally stable; a list of professional liability claims and/or legal actions within the last ten years including bankruptcies, ownership changes & a non-binding fee proposal.
