Description:

The Department of Human Services (DHS) is seeking contractors to provide Spiritual Supports services to individuals in selected facilities. These facilities are listed on Attachment A.
This Invitation for Bid will result in a DHS Multiple Award Contract for qualified vendors to provide Non-Denominational Christian Chaplaincy Services (other than Jewish, Catholic, and Muslim) for DHS facilities based on the attached Statement of Work.

Bids are due on April 16, 2018 at 11:00 AM and must be submitted electronically. Late bids will be disqualified. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.
Bidders will provide their cost by entering their per unit cost; a unit is described as EA which is equal to 1 Hour of Spiritual Support Services for the designated unit on each line for each year of the contract. Units are defined in the Statement of Work and will include ALL attenant costs including travel. Travel should be calculated in accordance with the Commonwealth Management Directive 230.10.
Supplier must enter firm prices for all contracted years.
All forms which requested to be returned are to be attached to the vendor submission must be complete for the bid to be accepted as responsive.
