Community-Based Health Care Program
Description:
The goal of this program is to expand and improve health care access and services, reduce unnecessary utilization of hospital emergency services and to encourage collaborative relationships among community-based health care clinincs, hospitals and other health care providers. This solicitation offers five categories of funding options. Category 1 is for the development of a new community-based health care clinic. Category 2 is for the expansion of primary health care services at an existing community-based health care clinic. Category 3 is for the addition or expansion of prenatal, obstetric, postpartum and newborn care services at an existing community-based health care clinic. Category 4 is for the development of alternative health care delivery systems administered by community-based health care clinics which improve services and access to reduce hospital emergency room utilization. Category 5 is for the implementation of collaborative relationships to enhance transitions of care for patients to ensure timely follow-up care for health care clinic patients seen or admitted to a hospital and to develop referral mechanisms to establish a health care medical home for patients seen in a hospital and specialty clinics without a regular source of primary health care.
