Description:

The Department of Human Services,
Torrance State Hospital is soliciting a
supplier to provide bread items as
described in the attached specifications.
Bids must be submitted electronically.
This is for the delivery of Breads for the period of JANUARY - MARCH 2023.
Quantities listed are estimates and may increase or decrease based on the needs of the facility.
Bid awarded on low total for all items.
Bidders will only bid the acceptable brands, pack size and codes as specified. all items to be in accordance with pa specifications that are attached.
Dietary Dept. will notify vendor on quantities needed for each delivery date.
Alternate deliveries may be arranged, as required. Quantities may be adjusted at the request of the Dietary Dept, to meet the needs of our consumers.
A response must be given for each attribute.
Complete and return your bid electronically.
Attach your Reciprocal Limitations Act Requirements Form to your bid. The Attached Reciprocal Limitations Act Requirements Form needs filled out in its entirety and submitted with your bid, ALONG WITH NEW WORKER PROTECTION AND INVESTMENT CERTIFICATION FORM.
Attachments to the Reciprocal Form will not be accepted.

Failure to submit bid as described above, may result in your bid being rejected as non-responsive.
Deliveries are Monday through Friday from 8:00 am until 11:45 am.
