The Department of Human Services (“DHS”), Office of Mental Health and Substance Abuse Services (“OMHSAS”) requires an Optometrist to provide eye examinations for refractions and slit lamp exams to meet the rehabilitative needs of the patients at Torrance State Hospital (“TSH”), State Route 1014, Torrance, PA 15779, Westmoreland County, Pennsylvania. The selected Contractor must provide a qualified optometrist to perform the examinations services, including routine eye examinations, eye refractions, and slit lamp exams, as well as Diabetic and Seroquel exams on site at TSH. The selected Contractor shall establish a schedule, at least four weeks in advance for coming to TSH. The schedule must be mutually agreed upon by TSH and the contractor to meet good patient care. If changes in the schedule are required, Contractor shall notify the Nursing Office.
The term of this Service shall commence upon issuance of a Purchase Order (“PO”) to the selected Contractor (“Effective Date”) and shall expire up to one year after the Effective Date unless it is terminated earlier pursuant to the terms of the PO. DHS shall have the option to extend the terms of the PO upon the same terms and conditions for up to four additional one-year renewal terms.
