Description: The Department of Human Services (DHS or Department) Office of Children, Youth and Families (OCYF) is seeking a contractor to provide general dental services to residents residing at the North Central Secure Treatment Unit (NCSTU) and Youth Forestry Camp#2 (YFC2). 

The awarded Contractor will provide services to YFC2 residents at the Contractor’s place of business. Contractor shall be within a 20-mile radius from OCYF’s location. 

The awarded Contractor will provide services to NCSTU residents at North Central Secure Treatment Unit, 36 Kirkbride Drive, Danville, PA 17821.  In the case of a need for emergency services, the awarded Contractor may be required to be examine residents at the Contractor’s place of business.

The term of the contract will commence upon the full approval of the awarded contract and will be for a term of 3 years. The Department may extend the contract on the same terms and conditions for two single year periods.

Suppliers must enter firm prices for all contracted years.

Bids are due 7/10/19 by 10:00 AM and must be submitted electronically.  Late bids will be disqualified.  Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.
