Description:

The Department of Human Services, Office of Mental Health and Substance Abuse Services, Warren State Hospital is seeking a contractor to provide a Licensed Psychologist Director for consumers at Warren State Hospital located at 33 Main Drive Warren, PA 16365.
Suppliers must enter firm prices for all contracted years.
Services to be performed are listed in the Statement of Work attached to this Solicitation.
Suppliers are required to complete the contact information on the attached Statement of Work and attach to their bid response.
Bids are due on January 21, 2019, 11:00 AM and must be submitted electronically. Late bids will be disqualified. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.

Quantities listed are estimates and may increase or decrease based on the needs of the facility.
