Description: The Department of Human Services, Office of Mental Health and Substance Abuse Services, Warren State Hospital is seeking a contractor to provide Part Time Physical Therapy Services for consumers at Warren State Hospital located at 33 Main Drive Warren, PA 16365.

Suppliers must enter firm prices for all contracted years.

Services to be performed are listed in the Statement of Work attached to IFB in the SRM portal.
Suppliers are required to complete the contact information on the attached Statement of Work (in the SRM portal) and attach to their bid response.

Bids are due on September 24, 2018, 11:00 AM and must be submitted electronically.  Late bids will be disqualified.  Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.
