Description: The Department of Human Services, Ebensburg Center (EBC) is seeking a contractor to provide Per Diem Direct Support Professional /Residential Service Aide Services for consumers located at 4501 Admiral Peary Hwy, Ebensburg PA in Cambria County.

Ebensburg Center is an Intermediate Care Facility for individuals with an intellectual or physical injury or illness, dysfunction behavior and/or developmental disability. EBC has a bed capacity of 402 individuals with a current census of 211.

Anticipated Term of the contract will be August 1, 2018 (or upon full execution of the contract; whichever is later) through September 30, 2019. 

Suppliers must enter firm prices for all contracted years.
Bids are due July 5, 2018, 11:00 AM and must be submitted electronically.  Late bids will be disqualified.  Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.
