Description:
|
The Department of Human Services, Selinsgrove Center is soliciting for a supplier to provide an optometrist, eyeglass dispensing, and eyeglass repair services to individuals (approximately 221) on site at Selinsgrove Center- per the attached Statement of Work.
Both parties will agree upon number of sessions per contractor visit. Appointments will be scheduled at mutually agreeable times to both parties.
Suppliers must enter firm prices for all contracted years.
