Description:
|
The Commonwealth of Pennsylvania, Department of Conservation and Natural Resources, Michaux State Forest, requires services to deliver, paver-place and roll approximately 5,700 tons of DSA (driving surface aggregate), to Stillhouse Hollow Road, Michaux State Forest, Franklin County. Please refer to the Statement of Work and attachment for specifics.
Please note a few important items:
1. Bidder must include Reciprocal Limitations Act form that lists the state of manufacture for any supplies procured.
2. Liquidated damages can be assessed should the vendor have specific performance issues. Please refer to Section III of the Statement of Work for specifics.
3. The vendor must include with their bid a properly executed and signed DCNR Bureau of Forestry DSA certification Form (attached) stating the surface material DSA meets the required criteria.
4. Bidder is to provide name, email address and a phone number of a company contact person.
