Description:
Wernsersville State Hospital is seeking a contractor(s) to provide numerous outpatient services as defined in the SOW. The attached SOW is rough draft 1. This contract term will serve from July 1, 2015 through June 30, 2020. This will combine the following contracts: 4400008275 expires 6/30/15, 4400008269 expires 6/30/15, 4400008827 expires 8/31/15, 4400008826 expires 6/30/15, 4400007824 expires 10/30/15 and 4400008274 expires 6/30/15. The facility would like this to be bidded out with MA rates.
