Description:
The Pennsylvania Department of Health is soliciting applications to implement the Pennsylvania Do You cAARd? Ask, Advise, Refer (PA cAARds) training curriculum statewide in each of the community health districts and for evaluation of this initiative. PA cAARds is a statewide cross-training curriculum and resource integration program for health care and community-based organizations providing diabetes and/or tobacco control services. This initiative will focus on the training of diabetes educators, who will refer patients to tobacco cessation programs and will also cross-train tobacco prevention and cessation specialists in order to refer patients to diabetes education and/or self-management programs.All Bids must arrive prior to the Bid opening date and time to be considered, and become property of the Commonwealth once submitted.
