Description:

The PA Dept of Health is soliciting education and training applications for Improving Cardiovascular Health Literacy from PA institutions and organizations. The overall goal of this funding is to improve the cardiovascular health literacy of persons with inadequate or marginal health literacy skills, focusing on low-income persons, ages 50 years and older, as well as health care providers and systems, as they impact heart disease and stroke. Emphasis shall be placed on improving the health care provider and patient encounter for increased patient understanding of his or her cardiovascular medical condition and management thereof. The applicant shall use recommendations from the Joint Commission report, "What Did the Doctor Say?: Improving Health Literacy to Protect Patient Safety," to make changes in a health care system related to health literacy. Applicants are expected to develop, establish, implement, and evaluate programs that target health care sonsumers, health care providers and systems to mitigate the negative effects of low health literacy. All Bids must arrive prior to the Bid opening date and time to be considered, and become property of the Commonwealth once submitted.
